PARENT/GUARDIAN CONSENT

I confirm I am the athlete's parent or legal guardian, or I am authorized by the parent/legal guardian to submit this registration and provide the athlete's information.

LIABILITY WAIVER

I (Parent/Guardian) am the parent or legal guardian of the registered child. As lawful consideration for being permitted to participate in Faith & Grind Basketball, I agree that I will not make a claim against, sue, attach the property of, or prosecute Faith & Grind Basketball for damages for death, personal injury, attracting the Covid-19 Virus, or property damage which I may sustain as a result of participation in basketball activities.

This release is intended to discharge Faith & Grind Basketball and its employees from any and all liability, including negligent actions, arising out of or connected in any way with participation in Faith & Grind Basketball except for liability arising out of willful or excessive misconduct. I further understand that sports involve physical contact between players and that serious accidents occasionally occur. Knowing the risks, I agree to assume those risks and to release and hold harmless Faith & Grind Basketball Camp/Clinic and its employees.

I have carefully read this agreement and fully understand its contents. I am aware this is a release of liability and a contract between Faith & Grind Basketball Camp/Clinic and I have signed it of my own free will.

MEDIA RELEASE

I authorize Faith & Grind Basketball School to photograph, record, and use the athlete's name, image, likeness, voice, team photos, video, and program participation in website, social media, email, print, advertising, and other promotional materials for Faith & Grind Basketball School.

I understand this permission is voluntary but required for online registration at this time. I represent that I have authority to grant this permission for the athlete.
